| The form must be
accurately completed and filed at registration. There will be a $30 entry fee if
you register before June 20, 2008. If you register after June 20th, the fee will
be $35 per individual. |

| Team Awards: For the fifth annual Midwest
Nationals, awards will be presented to the top four teams in each division. A maximum of 12 wrestlers
can be counted towards team scores.

| Attention U.S. Veterans: Free coffee and donuts will be available for U.S. Veterans from 7:00am-10:00am
on July 12 inside the Shirk Center. |

| Awards: Awards
will be given to the top 6 individuals in each weight class. Team trophies will
be given to the top 4 in each class. |
| Uniforms: Must meet National Federation Rules. Headgear
is required. |
| Medical
Information: Athletes must be prepared and must submit to a Skin
Disease screening at weigh-in. The Chief Medical Officer’s decision is
final and not subject to appeal. |
| Weigh-Ins: All athletes will be required to weigh-in
once on Friday, July 10th from 5 p.m. - 8 p.m. |
